The “Benadryl Challenge” is a dangerous social media trend, primarily on TikTok, that dares teenagers to ingest 12 times the normal dose of diphenhydramine to hallucinate and film it. Originally starting in 2020, the trend has seen a severe resurgence in 2026, with overdose cases more than doubling in the first five months of the year compared to the same period in 2025. Fatalities are most frequently reported in youth aged 14-18, including children as young as 13. Diphenhydramine is a common active ingredient found in many household over-the-counter (OTC) medications, including Benadryl, ZzzQuil, Nyquil, Unisom SleepTabs, Dimetapp, Tylenol PM, Robitussin, Advil PM, Children’s Triaminic, and various store brands.

Signs of a Diphenhydramine Overdose

Families should monitor for the following severe symptoms:

  • Extreme agitation or confusion.

  • Hallucinations or delirium.

  • Inability to urinate paired with hot, dry skin.

  • Rapid or irregular heartbeat.

  • Seizures.

  • Loss of consciousness or coma.
